1. Is there a list somewhere that breaks down what polity the "independents" from the first PSB belonged to, at first?

Most independents were just that during the SL era- the only exceptions are Herotitus which was a joint colony of the MOC and TC (although that may have been retconned) and the Oberon Confederation, Von Strang's World, Circinnius, Elyssia, Butte Hold & the Valkyrate which were part of the RWR

2. Is there any word on creating post-Coup Periphery states?

I'll have more details when I get around to FM: P 2812.  I haven't really worked it all out yet.
